Description:
Provide comprehensive primary care services to adult Veterans
Diagnose, treat, and manage acute and chronic medical conditions
Perform preventive care, health screenings, and wellness counseling
Manage ongoing care for conditions such as diabetes, hypertension, heart disease, and COPD
Coordinate care with specialists, nurses, social workers, and other interdisciplinary team members
Utilize VA electronic health records for documentation and care coordination
Ensure compliance with VA clinical guidelines, quality standards, and patient safety policies
Participate in quality improvement, peer review, and performance initiatives
