Description:
Coordinate and manage patient care through VA telehealth programs
Conduct remote patient assessments via phone, video, and digital monitoring tools
Educate Veterans on disease management, medications, and use of telehealth equipment
Monitor patient data and clinical alerts from home telehealth devices
Collaborate with primary care providers and interdisciplinary healthcare teams
Develop, implement, and evaluate individualized care plans
Document clinical encounters in VA electronic health records
Ensure compliance with VA policies, patient safety, and privacy standards
Support chronic care management, prevention, and follow-up services
