Description:
✔ Provide comprehensive hospice and palliative care to Veterans in both inpatient and outpatient settings.
✔ Work collaboratively with the interdisciplinary Palliative Care team — including nurses, social workers, chaplains, and other allied professionals.
✔ Manage complex care needs, symptom control, and comfort-oriented treatment plans.
✔ Conduct inpatient consults, outpatient evaluations, and possibly telehealth consults for Veterans unable to travel.
✔ Provide leadership, education, and mentorship (including training residents/fellows when needed).
✔ Collaborate with Home-Based Primary Care and other clinical services as necessary.
