Details of the Service
Damien House Regional Services (DHRS) provides community-based residential care and day services for adults with moderate, severe, and profound intellectual disabilities, including individuals with behaviours of concern and a range of physical, mental health, and complex medical needs. Services are delivered across community residential settings and day centres and operate within a structured governance framework led by Clinical Nurse Managers (CNM2/CNM1) and Social Care Leaders.
The local Interdisciplinary Team includes specialist input from a Clinical Nurse Specialist in Behaviour Support and a Senior Clinical Psychologist.
DHRS adopts a biopsychosocial model of care, delivering person-centred supports within community-based residential homes and day services. Each community home is directly managed by a CNM2/CNM1 or Social Care Leader, with overall service management provided by the Regional Social Care Manager and the Director of Nursing.
DHRS comprises four centres within CHO 5:
Damien House Services, Clonmel, Co. Tipperary
Avila, Fethard, Co. Tipperary
Re Nua, Cashel, Co. Tipperary
Leeside, Callan, Co. Kilkenny
Purpose of the Post
The Social Care Worker will work as part of a multidisciplinary team to deliver high-quality care and support to residents, under the supervision of the Clinical Nurse Manager (CNM2/CNM1) or Social Care Leader.
The primary responsibility of the role is to provide direct and indirect day-to-day care to residents, supporting their health, wellbeing, independence, and quality of life. The Social Care Worker will contribute to the delivery of a person-centred, rights-based service, enabling residents to live meaningful lives within the community.
