To provide leadership and coordination within the multidisciplinary disability team, ensuring the delivery of integrated, person-centred, and rights-based services. The Team Leader is responsible for overseeing care planning, fostering effective interagency collaboration, and supporting staff in the application of trauma-informed and strengths-based practices. The role ensures that the voices of service users are central to decision-making and that services are delivered in accordance with best-practice standards, governance requirements, and safeguarding frameworks.
