Reporting Relationship
The Community Healthcare Network Manager reports to the Primary Care General Manager, who holds overall accountability and responsibility for the delivery of primary care services within the defined network area. The postholder will work in close collaboration with Heads of Discipline and colleagues across Primary Care to ensure coordinated and effective service delivery.

To support the effective implementation of the Network Operating Modelβfocused on multidisciplinary collaboration, service coordination, and integrated careβthe Community Healthcare Network Manager will maintain strong working relationships with:
β’ GP Lead
β’ Community Nursing
β’ Heads of Discipline
β’ Primary Care staff assigned to the Network
The Community Healthcare Network Manager and the Heads of Discipline together form the core leadership necessary for the safe, effective, and high-quality provision of services across the Network.
Purpose of the Post
The Community Healthcare Network Manager is accountable and responsible for ensuring the delivery of high-quality, safe, and coordinated primary care services to the population of the Network. These services must be delivered in accordance with statutory requirements, relevant policy and service delivery frameworks, and within allocated resources.
Operating through a multidisciplinary, team-based model of care, the Community Healthcare Network Manager will:
β’ Hold full responsibility and accountability for managing designated Primary Care staff within the Network.
β’ Provide day-to-day operational and line management to these staff.
β’ Work collaboratively with Heads of Discipline, who retain responsibility for clinical governance, professional standards, and clinical supervision of their discipline-specific staff.
β’ Ensure adherence to quality, safety, and best-practice standards across all professional activities.
The postholder will be responsible for coordinating the integration of community healthcare services within the Network, ensuring responsiveness to population needs and service delivery requirements. In addition, the Manager will lead on service user engagement and community involvement, fostering strong links with local communities to support person-centred care.
