Description:
The Registered Pharmacist – Hospital in the Home (HiTH) provides clinical pharmacy services to patients receiving acute and subacute care in their own homes. This role supports medication management, antimicrobial stewardship, IV therapy coordination, patient education, and collaboration with multidisciplinary HiTH teams to ensure safe, effective, and patient-centred care outside the hospital setting.
