Description:
Provide clinical case management and coordinate care for homeless Veterans.
Support programs such as Grant and Per Diem (GPD), Contract Residential, HUD-VASH, and Veterans Justice Outreach (VJO).
Participate in community outreach to identify and engage Veterans who could benefit from HCHV services.
Assess Veteran eligibility for VA medical care and homeless services.
Coordinate care by linking Veterans to VA and community resources and assist with achieving housing stability.
Maintain required documentation and follow local HCHV policies and procedures
