Description:
(1) Evidence of current professional licensure, certification or registration to practice in the State of Alabama for each provider of service and to notify the Department immediately of any pending Licensure Board action(s).
(2) Evidence of professional liability insurance for each provider of service.
(3) Employment Eligibility Verification (Form I - 9) for each provider of service.
(4) Validation of competency as determined by Department policy utilizing the appropriate Competency/Skill Appraisal form for each Provider of service.
(5) Initiation of the Employee Health Statement toward the completion of required tests and training as described in Department policy for each provider of service.
(6) Current validation of competency in Emergency Cardiopulmonary Resuscitation for each provider of service.
