Description:
Screen Veterans for transfer or admission to residential programs across Veterans Integrated Service Network (VISN) 10.
Conduct psychosocial assessments to evaluate needs and determine the least restrictive appropriate setting.
Complete screenings, follow-up calls, and assist in coordination of placement into treatment programs.
Manage and update data systems related to the Mental Health Residential Rehabilitation Treatment Program (MHRRTP).
Collaborate with VA providers, community agencies, Veterans, and caregivers to ensure coordinated care.
Provide support and intervention for Veterans awaiting admission, including mental health or substance abuse screenings and possibly brief therapeutic interventions.
