Description:
Support delivery of clinical care through telehealth platforms
Conduct remote patient monitoring and follow-up calls
Collect and document vital signs, symptoms, and patient-reported data
Educate Veterans on use of telehealth equipment and health management
Assist with chronic disease management and preventive care activities
Escalate clinical concerns to Registered Nurses or providers as appropriate
Coordinate appointments and telehealth visits
Maintain accurate documentation in VA electronic health records
Follow VA policies related to privacy, telehealth, and patient safety
