Description: The pharmacist reviews and verifies prescriptions, ensuring correct dosages and identifying potential risks. Your key responsibilities are: Review each patient’s medication chart or prescription for the appropriateness of the drugs prescribed and to contact the prescribing doctor when an intervention is needed. The review ensures that all orders are clear and specific doses are prescribed within therapeutic limits. Any potential adverse reactions and interactions are identified. Fulfill all drug orders accurately and in a timely manner. Check / verify prescriptions and dispenses prescribed drugs with counselling. Performs/ oversees in-patient and out-patient discharges. Provide drug information services to medical staff. Prepare extemporaneous products as and when prescribed Maintain proper inventory records on all drugs. Conducts stock check to ensure that the stock in all Wards is adequate and replenishes stock, when necessary.
