Description: You will work within inpatient, outpatient, ward-based and dispensary areas, supporting safe and effective medication management across the hospital and broader health service. Deliver a wide range of clinical pharmacy services including dispensing, medication histories, reconciliation, Medication Action Plans, Pharmaceutical Care Plans, counselling, drug information, and TDM.
