Description: You shall provide collaborative, professional and patient-centered clinical services under supervision. Prepare for and participate in multidisciplinary (MDT) rounds to review patients’ clinical status and treatment needs holistically, address drug queries and supply issues, identify and manage drug-related problems, e.g. optimize drug therapy and monitoring. Listen to, engage, and empower patients and caregivers with needful medication counselling and disease management advice. Continuously develop oneself professionally through effective learning, testing, reflections and constructive feedback. Work effectively with a team of Pharmacists, Pharmacy Technicians and Pharmacy Assistants in the provision of safe and high-quality pharmaceutical care services.
