Description: Responsibilities Provide health assessments, screening, counseling, health education, and other nursing services at the District Health Centre (DHC) Core Centre and Satellite Centres Participate actively in health promotion initiatives and primary prevention programs within the community Act as a case manager for community-based rehabilitation programs, ensuring continuity of care Coordinate patient care among multidisciplinary teams within DHC and across community network service providers Utilize and maintain the clinical management system for accurate clinical documentation and effective communication
