Description: Job Requirements We are seeking a highly skilled and experienced Clinical Pharmacist to join our team at Polakulath Narayanan Renai Medicity in Kochi. As a Clinical Pharmacist, you will be responsible for dispensing medications, providing drug information to patients and healthcare professionals, and ensuring safe and effective use of medications. You will also be responsible for managing inventory, conducting drug utilization reviews, and participating in patient care rounds. Qualification: Bachelor's degree in Pharmacy and valid state license Key Responsibilities: - Dispense medications accurately and efficiently - Provide drug information to patients and healthcare professionals - Conduct drug utilization reviews and make recommendations for improvement - Participate in patient care rounds and provide medication therapy management - Manage inventory and ensure proper storage and handling of medications - Collaborate with healthcare team to ensure safe and effective use of medications - Stay updated on new drugs and therapies and provide education to staff and patients - Maintain accurate patient records and documentation Qualifications: - Bachelor's degree in Pharmacy from an accredited institution - Valid state license to practice as a Pharmacist - Minimum of 2 years of experience as a Clinical Pharmacist - Strong knowledge of drug therapy and medication management - Excellent communication and interpersonal skills - Ability to work in a fast-paced and dynamic environment - Attention to detail and accuracy - Strong problem-solving and critical thinking skills
